Accounts that are inactive, will move to inactive status after 180 days and will not be accessible to the end user. To resolve this, please send us an email to records@healthmedicalmanagement.com.

Our practice does not accept any records request by fax, email, and personal service. You must upload your request for records from our secure Request Upload Module. If you are unable to upload, please mail your request to our Request Processing Center located at 1142 S. Diamond Bar Blvd #310, Diamond Bar, CA 91765.

Please make sure the following is included:

  • Your request for records needs to be served —with an initial processing fee of $15.
  • Our doctors provide treatment at different medical locations —The treating doctor's name and address must be included in your request.
  • Your request must be directed to "Health Medical Management".
  • We required individual requests for Medical, Therapy, Billing, X-Rays, etc records —as each file item is retrieved and processed separately.

There is no need to spend the time, money on gas, or postage fees to serve us your request for records. Upload your request for records, it's faster, easier and convenient. Please upload your request from our secure Records Request Upload Module.

If you are unable to upload from our secure Records Request Upload Module, please mail your request for records to our Request Processing Center, located at: 1142 S. Diamond Bar Blvd #310, Diamond Bar, CA 91765. If you are not a registered user, please sign up today.

All requests are subject to twenty (20) business days turnaround. However, some requests could be completed sooner, depending on the chart situation.

You can always check the status of your request from our Online Status Portal, all you need is your unique Reference Number and the Patient's Date of Birth.

As soon as your request is complete, we will send you an email notification. You can download your file as many times as you need for ONLY 5 FIVE DAYS, from the notification date.

After this time frame, your file is no longer available. If for whatever reason we need to re-upload a file, there will be a re-upload charge fee.

You can always check the status of your request for records from our secure Online Status Portal, all you need is your unique Reference/ Order Number (exactly as provided on the request) and the Patient's Date of Birth.

All requests are subject to twenty (20) business days turnaround. Please do not call during this period of time. We'll keep you posted on any further updates by fax and email. As soon as your request has been served properly, you can contact Support Team for any questions regarding records.

Avoid processing delays, do not serve our medical practice, our medical staff will not process your request for records. PLEASE update your database and mail your request for records to our Support Team.

You can submit a digital payment for your Initial Processing Fee from your secure online portal account. However, at this time our practice does not accept digital payments for our Final Reproduction Cost.
